02Can I use this to split a headphone output to two amps?

It will physically split left and right into two mono plugs, so yes for that purpose. It won't duplicate one signal to two destinations.

03Do I need one per channel?

Yes. Each insert jack takes its own Y-cable, so stereo or multiple channels means buying multiples.

04Will this work with a TS insert jack?

No. Insert points require a TRS jack to carry send and return on one connector. A TS jack has no ring to split.
